Rise in Unity CIC accepts referrals from schools, probation services, local authorities, and parents/guardians. Please complete the referral form below to request a placement in one of our programmes, including Alternative Provision, Youth Hub, Martial Arts, or Community Projects.

You will need:

• Young person’s details (name, DOB, contact information)
   
• Parent/guardian contact information
   
• Reason for referral and support needs
